The forms of court that are included in the Rules of Court are officially published online under the authority of the Queen’s Printer Act here: Forms. These official forms are in a Portable Document Format (PDF) that cannot be edited on a computer.

The following forms of court are provided in Microsoft Word format for convenience. Although the Court’s staff strives to maintain the accuracy of these forms, they are provided without any warranty.

These forms are reproduced with the permission of the Queen’s Printer for New Brunswick. This is not the official version of these forms. In the event of any inconsistency between this version and the official version that is published under the authority of the Queen’s Printer Act, the latter prevails. However, the unofficial forms in Word format have been amended following the demise of Her Majesty Queen Elizabeth II, on September 8, 2022, to reflect the change in the Crown’s designation. Wherever they appear on the official forms, the name of the Court of Queen’s Bench and the designation of Her Majesty the Queen have been changed to the Court of King’s Bench and His Majesty the King, respectively, on the Word format forms provided below.

The forms may be saved to a computer and completed using a word processor. When printing a form:

  • use quality letter size printer paper (216 mm x 279 mm or 8.5” x 11”), and
  • print only on one side of the paper.
